i think that after a survivor (not zombies because they don't care) drops to less than 20 Hp they should start to lose 1hp per an ap like infection. This could also be cured with a med pack.
(this is relistic and would even the scales bettween zombies and humans)

Maybe bleeders could leave a trail.
There is a trail of blood leading west/south/inside

New Item for it:
Bandages Having a bandage in your inventory automatically stops bleeding. Can alos be used on other survivors.

Hrmm, interesting. Though, loosing HP might be a little.. drastic, because it penalises you for being at low health. Does this damage happen instead of an infection, if their allready infected, or do infected bleeders loose 2 hp a turn?
I like the trail idea. Heh, hell, i like the idea in general =P
EDIT: Bandages sound good, too ^^

yeah i like the idea but you only loss 1 hp when you move to a building and every class starts out with 1 or 2 depending on what they are
